Order Form for Gilded Personalized Diploma #3

Please give the following information, exactly as you want it to appear on the diploma:
(For accuracy, we copy and paste from this information, so please be precise and do not use all capital letters!)

1. The name of the high school We will NOT add the word "school;" type the name exactly as you want it to appear. (For example, enter "Wilson High School" instead of Wilson High") If you do not have a school name, or if your school name is unusually long or includes an acronym, please read this. Please don't waste your time and ours by typing in the name of a public high school. We do not fill such orders.
2. Optional: City and state of school This information is not always found on a High School diploma, but is an option which we recommend. If selected, city and state will be placed below the school name in a script font. Alternately, this space may also be used for a school motto, if you have one. (Maximum 40 characters and spaces.) (Leave this field empty if you do not want either)
3. The name of the Graduate: Full names (rather than initials) are normally used on a diploma.
4. Gender of graduate: Male Female
5. The date on the diploma. If you give us "May 22" for the date, the diploma will read "dated this twenty-second day of May..." etc. However, if you prefer to have only the month and year on the diploma, you may select "no specific day" for the date. (Without a day listed, the diploma would follow this format: "Dated this month of May...." etc.) Month: Date: Year: (Note: After making your selection from the drop down menu, be sure to click outside the box to "lock in" your choice. Otherwise, you may mistakenly change the data when you use the scroll button on your mouse.)
6. Indicate your choice of wording: The diploma you are ordering reads "It is the culmination of his/her High School education, a milestone at which we recognize...." etc. Would you like to:
		Leave it as written - "It is the culmination of his/her High School education..." OR
		Change it to read "It is the culmination of twelve years of Christian education..." OR
		Change it to read "It is the culmination of twelve years of home education..."
7. Choose the seal design: 23kt gold will be applied to the seal and also to the initial letter of your school name. (This is seal goes in the center of the diploma between the signature lines. A small, rectangular "honors seal" may also be added to the diploma: they are offered below.)

8. Indicate the Bible verse of your choice: (There is no extra charge to use one of our standard verses. If you want to use your own Bible verse there is a $3.99 charge.)

Custom Bible verse: Write your verse in the box below, deleting the word "none." We will print it exactly as you give it to us. Maximum number of characters:180. The field below will show 100 characters and spaces, but will allow you to enter up to 180.This option adds $3.99 to the cost. Note: The script font used for the Bible verse does not have Roman numerals. If you are using a book like "II Peter" you should choose to use the numeral ("2 Peter") or write out the name of the book like this: "Second Peter." Please don't give us the Roman numerals, or your order will be delayed while we take the time to contact you. Also: words written in all capital letters, such as "LORD" or "NKJV" can be problematic in the script font. The capital letters have flourishes that can overlap, making them hard to read. We will put it on the diploma the way you give it to us, but we ask you to consider this information when you type your verse.
9. Select the Lettering style for the student's name: (see samples)

		There is a $5.50 extra charge for hand calligraphy, and it adds one day to the production of the order.
10. Give us the titles that go beneath the signature lines: Because you are issuing the diploma, you will sign the signature lines yourself: this is for the titles that are placed under the lines. If there will be only one person signing the diploma, we add a line for the signer to write the date that they signed it. This simply makes it look more balanced. If you need this arrangement, chose the third option. If you don't see the signature line option that you want, select any one, submit your order, and then follow the directions below for a wording change. Sign your student's diploma with this lovely rosewood ballpoint pen and then give it to them as a keepsake! Beautiful finish, engraved with "Class of 2009" and a cross. Barrel twists to extend the nib. (Sorry, not available for 2008 or prior years.) This is a stocked item that will ship with your diploma. Limit ONE per diploma. 17. RUSH Option: (more info about the production time for diplomas )
		Yes, add the RUSH option to the production of my diploma order. (adds $10.00) If this box is not selected, the regular production time will apply. Regular production time on a Personalized diploma is three working days (not counting the day the order is placed) PLUS one additional day for each of the following options: hand calligraphy, diploma cover personalization. See this page for more information about production time and the RUSH option.
NOTE: The RUSH applies to the diploma order and any stock items that would ship with it from our office. (Stock items are those listed as leaving our facility in 1 - 2 business days.) The RUSH does not change the shipping time on items that require a longer production time, such as name cards, address labels, personal notes,etc. If placing a RUSH diploma order, any items with a production time longer than 1-2 business days should be placed as a separate order, or they will hold up the diploma.
18. Name of person placing order: By entering my name in the field below, I verify that I am the administrator of the school listed above, and that I have the authority to issue this diploma. I understand that Homeschool Diploma.com is not an accrediting agency or school and does not "issue" diplomas or verify the information on the diplomas, but provides a graphic arts and printing service to school administrators who wish to issue a diploma. I understand that all claims made on the diploma are my responsibility.
